Sam's POV is complicated because he was a kid--his view of Dean was going to undergo huge changes no matter what. He doesn't attribute any of those changes to Dean's looks, but he might be wrong. In general, I expect that Sam thought all those things, at different times, and swung back around to "delinquent" as he became a teen himself and started to want a different life.
In my head, Dean thinks his body is a tool, not for getting love but for getting stuff done, and being good-looking makes it a more effective tool. I don't think he thinks his family loves him more, but I think he thinks he's less unworthy. Looks would never be enough to satisfy his dad, and he knows that.
